Release of Liability and Consent Agreement
1. Liability Disclaimer:
The tennis training is operated by Bajoit Tennis Academy and is not affiliated with Westwood High School, Southwestern University, or their brands. Participation in the training is undertaken at your own risk. Bajoit Tennis Academy, its staff, coaches, and affiliates will not be held liable for any injuries, accidents, illnesses, or other incidents that may occur during training sessions, whether on or off the premises.
Participants are strongly advised to consult with a licensed medical professional prior to engaging in any physical activities, particularly if there are pre-existing medical conditions or concerns that may affect their ability to safely participate.
By registering for the training, you expressly acknowledge and accept the inherent risks of participation and agree to indemnify and hold harmless Bajoit Tennis Academy, its staff, and affiliates from any claims, liabilities, or damages arising from your participation in the program.
2. Emergency Protocol:
In the event of an emergency, participants or guardians are instructed to contact 911 immediately and notify the Bajoit Tennis Academy staff at the earliest opportunity. Bajoit Tennis Academy reserves the right to take necessary actions in response to emergencies, including notifying medical professionals or emergency contacts provided during registration.
Additionally, I hereby grant permission to Bajoit Tennis Academy to use any photographs or video footage taken of my child during the tennis program for promotional purposes. This includes, but is not limited to, the use of such images in brochures, websites, and other marketing materials.
I understand that my child's image may be used in a manner that highlights his/her involvement in the tennis program and may also be used to promote future programs offered by Bajoit Tennis Academy. I also understand that no royalty, fee, or other compensation shall become payable to me by reason of such use.
3. Program Policies:
Bajoit Tennis Academy reserves the right to cancel or reschedule training sessions as needed. In such cases, credits or refunds may be issued at the sole discretion of the academy. Participants are required to adhere to all academy rules and policies, and failure to do so may result in suspension or dismissal from the program without refund.
This agreement shall be binding upon me, my heirs, assigns, and personal representatives.
