Note: To be accepted, photos must meet outlined requirements. If easier, Willow Glen Electric will be on-site in late April/early May to inspect and take photos of sub-panels (this service is provided at no cost to members). Members MUST verify their panel via this form or be available for inspection by Willow Glen. Thank you!

**Regardless of what type of panel you have (Zinsco or upgraded), upload a photo of your open electrical sub-panel here.** Photos must be clear, with the panel door open, the manufacturer label and your unit number (on a sticky note) visible. *Photos that do not meet these requirements, or which have been edited, will not be accepted by our insurer.
