I, the undersigned parent or legal guardian, hereby grant permission for my child to participate in the arts and crafts party hosted by Stefanie’s Art Studio. I understand that I am leaving my child under the supervision of the workshop staff during the event.
In case of an emergency or if the need for communication arises, I can be reached at the following contact numbers:
I acknowledge that participation in the arts and crafts activities at Stefanie’s Art Studio involves certain inherent risks. These risks may include, but are not limited to, minor injuries, allergic reactions, and exposure to art materials.
Release of Liability:
In consideration of my child's participation in the arts and crafts party, I release and discharge Stefanie’s Art Studio, its owners, employees, and representatives from any and all claims, liabilities, demands, actions, or causes of action that may arise out of or be related to any injury, loss, damage, or illness that may be sustained by my child during the party.
Medical Treatment Authorization:
I authorize Stefanie’s Art Studio and its staff to seek and consent to emergency medical treatment for my child in the event of an accident, injury, or illness during the party. I understand that every effort will be made to contact me or the emergency contacts provided before any such action is taken.
I have read and understand the terms of this waiver form.
I grant permission for Stefanie’s Art Studio to use photographs or videos of my child taken during the event for promotional or educational purposes.
